Result description
The RE-SAMPLE project has developed an AI-powered digital health platform that integrates real-world clinical and patient data to support preventive and personalised care for individuals with complex chronic conditions, particularly chronic obstructive pulmonary disease (COPD). The platform includes predictive models, clinical dashboards, and a virtual companion that enables early risk detection, real-time monitoring, and patient self-management support.

R&D, Technology and Innovation aspects
RE-SAMPLE offers an eHealth solution integrating 12 KERs. Booster market analysis was done for the Virtual Coaching module, the Health Data Hub, and the Building block of Italian shared care facility. Next steps are preparing the Healthentia clinical dashboard and other high-TRL KERs for commercial exploitation and supporting research-oriented components through follow-up funding.
RE-SAMPLE components are highly customisable, allowing for the inclusion of new features based on specific client requirements.
The RE-SAMPLE solution, including predictive models, digital care pathways, and clinical implementation protocols, is designed for high replicability. The machine learning tools and care frameworks can be deployed consistently across multiple healthcare settings and patient populations, ensuring the same level of quality, accuracy, and patient outcomes each time. The modular and standardised design allows the service to be scaled to other regions, hospitals, or home-care systems in Europe, supporting broad adoption of preventive and home-based care while maintaining uniform standards and regulatory compliance.
RE-SAMPLE sustainability roadmap provides a comprehensive framework for ensuring the long-term viability of the project’s outcomes. It complements direct commercialisation and commercial licensing with open science dissemination and follow-up research projects.
